As per our recent correspondence in relation to our payroll review project, you have been identified as entitled to a back payment for monies owed to you.

So that we can backpay you, please complete the form linked below using the unique Nomination ID (provided in your email), and information and/or documentation listed below:

Nominated bank account, including name, BSB, account number.
Superannuation standard choice form, which must be signed and dated.
Superannuation fund Letter of Compliance.
