Caregiver back payment
As per our recent correspondence in relation to our payroll review project, you have been identified as entitled to a back payment for monies owed to you.
So that we can backpay you, please complete the form linked below using the unique Nomination ID (provided in your email), and information and/or documentation listed below.
Mandatory attachments:
- Copy of bank statement showing name, BSB, and account number only (we do not need to see transactions/balances) — or a formal letter from the bank providing the same information.
- Completed super choice form.
- Superannuation fund Letter of Compliance.
If you have a self managed super fund (SMSF) then the additional documents for the super fund are required:
- SMSF registration letter with compliance documentation (access SMSF compliance documentation from here by entering SMSF ABN).
- SuperStream Electronic Service Address notification (ESA is included in SMSF letter or in the bank document).
- Super fund bank account details letter from bank or bank statement.
