Hospital Avoidance Response Team

The innovative Hospital Avoidance Response Team (HART) at St John of God Midland Public and Private Hospitals is proving highly successful at reducing the re-admissions of high risk patients.

HART provides support to the hospital’s Emergency Department and inpatient teams in the assessment and management of patients with ambulatory care sensitive conditions and a history of frequent ED attendance and hospital re-admissions.

The team aim to enhance the assessment of care needs for patients and their families and carers of chronic disease, improve patient education and self-management of chronic diseases and improve the communication across the care continuum.

Ambulatory care sensitive conditions covered by HART include:

  • Chronic Obstructive Pulmonary Disease
  • Congestive Cardiac failure
  • Type 1 or 2 Diabetes Mellitus
  • Urinary Tract Infections
  • Cellulitis

The team liaises with key community organisations and can provide follow up home visits to encourage engagement with the care management plan.

HART comprises a clinical nurse, senior occupational therapist and senior social worker, who work together to provide inpatient and follow up care.

For more information, please contact HART on [email protected] or (08) 9462 4008.
