About My Admission
You can use My Admission to complete your admission forms online before a planned stay at a St John of God Health Care hospital.
You can also use it to submit your financial consent form, make payments, and book maternity appointments and classes.
Please note: from 1 September 2026 St John of God Mt Lawley Hospital will become part of the public health system. If your admission is planned for Mt Lawley after this date, please speak with your specialist to confirm the admission process.
Before you start
Before starting your online admission, please make sure you have the following information ready:
- The reason for your admission including the name of your procedure.
- The date of your admission or procedure.
- Whether you are having a day procedure or staying overnight.
You may also need:
- Medicare card
- private health fund information
- a list of current medication
- medical history
- pharmacy safety net, concession card or DVA Gold Card.
You will need a valid email address and mobile phone number to register. If you do not have both, please contact us on 1300 863 566.
My Admission
Start or continue an online admission
To keep your sensitive health information safe, My Admission uses multi-factor authentication. When you log in, we will send a verification code to your email address or mobile phone so we can confirm your identity.
If you already have an account, you will be asked to set this up the next time you log in.
How to complete your admission online
- If this is your first time using My Admission, select create user on the login page to register your details.
- Register using a valid email address and password. A mobile phone number is also required for identity verification and hospital reminders in the lead up to your admission.
- Complete the admission details section. This usually takes 10 to 15 minutes.
- Complete the clinical history section. This includes details of allergies, medications and past surgeries. This usually takes 10 to 30 minutes, depending on your medical history.
How long it takes
Registration usually takes 15 to 45 minutes, depending on your medical history. If you cannot complete the form in one sitting, you can save your progress by clicking the save icon and return later.
Helping someone complete their admission
Family members and support people are welcome to help a patient complete their online admission.
