St John of God Healthcare at Home provides end-to-end care for patients, extending beyond the hospital, delivering extra clinical support after discharge.

Having extra support at home can help reduce patients’ feelings of isolation and vulnerability as they receive high quality, safe care in familiar and comfortable surroundings.  

Our nurses and other health professionals provide the same high standard of care in the home as at St John of God Health Care hospitals.

We work closely with referring doctors and health services to develop person-centred care plans. Any concern or deterioration is flagged quickly with the patient’s treating doctor.

Most St John of God Healthcare at Home services are covered by a patient’s private health insurance.
