Giveaway rules

Opening and closing dates

All entries collected from survey entrants that opt into providing their details will be accepted between 9am, 1st of each calendar month, and 8:59am 1st of the next calendar month, unless otherwise stipulated in the individual survey (Submission Period).

After the Submission Period has concluded an entrant who has participated in the online consumer review survey will be selected at random and deemed the winner. The winner will be drawn at random from all survey entrants that opt into the giveaway and provide contact details within the Submission Period.

Who may enter

The only people who many enter the competition are:

• Australian residents currently living in Australia
• aged 18 years old or over
• not a current employee of St John of God Health Care
• resident of Western Australia, Victoria and New South Wales and applying within these states.

How to enter

An entrant who participates in the St John of God Health Care online consumer review survey and provides the information requested within the Submission Period will be entered into the giveaway. The entrant must provide answers in a minimum of one St John of God Health Care online consumer review survey and submit it with their contact details. Entries can only be submitted via the Qualtrics survey platform. A person will receive a maximum of one entry per survey completed. They must complete a different survey per entry. Multiple completions of the same survey will only count as one entry. Each entrant will also be required to provide contact details.

Conditions of entry

An entry to the online consumer review survey giveaway will only be valid if the entrant agrees to be bound by these terms and conditions.

Instructions on how to enter and other details contained within the online consumer review survey form part of the conditions of entry.

All entrants who provide information to enter this survey provide that information to St John of God Health Care through the Qualtrics surveys programs and not to Qualtrics itself.

St John of God Health Care reserves the right to verify the validity of entries received, and to disqualify any entrant who submits an entry that is not in accordance with these terms and conditions.

Entrants who complete Online Consumer Review Surveys may enter a giveaway each Submission Period. The first completed survey in any Submission Period will entitle the entrant to be included in that period’s prize draw.

Selection of the winner and prize details

The winning entry will be drawn at random from all entrants who participate in the online consumer review survey giveaway, while abiding by the rules of the competition.

The winner will be notified within one week of the close of any competition, via their contact details provided within the survey and will be given two weeks to contact St John of God Health Care to provide details of where to send their prize.  The winner will receive a $50 gift voucher, unless otherwise stipulated in the individual survey.

The prize must be accepted as offered and is not exchangeable, transferable or redeemable for cash.

If any aspect of the prize cannot be delivered for any reason, St John of God Health Care reserves the right to substitute the prize for any other item of equal or greater value.

If the prize remains unclaimed within two weeks of the competition closing, the prize will be considered forfeited and may be used by St John of God Health Care for an alternative purpose, entirely at the discretion of St John of God Health Care.

Privacy

Personal information collected by St John of God Health Care in conjunction with this competition will only be used to identify the winner of the competition and for St John of God Health Care’s future marketing and promotional activities.

All personal information collected by St John of God Health Care in conjunction with the prize draw will be handled and used in accordance with St John of God Health Care’s Privacy Policy, available at: https://www.sjog.org.au/patients-and-visitors/privacy

Limitation of liability

St John of God Health Care, its employees, agents or contractors will not be liable for any loss or damage whatsoever which is suffered (including but not limited to indirect or consequential loss) or for personal injury which is suffered or sustained, as a result of this prize draw or the prize, except for any liability which cannot be excluded by law. In the event that any such liability cannot be excluded by law, the total aggregate liability of St John of God Health Care will not exceed the total cost of the prize (being up to $50, unless otherwise stipulated in the individual survey).
