Mid West palliative care providers join forces to improve patient care

Patients will own the folders and bring them to all their appointments so that each health professional involved in their care will have access to a comprehensive, up-to-date shared care record. The folders have been enhanced by eye catching art work donated by local artists.
The patient’s family will be better able to participate in their relative’s care throughout the palliative care journey by having access to the information in the folder which remains with the patient throughout their care.
Grainne Friel, CEO of St John of God Hospital Geraldton said that this year, of our 75th anniversary of the Sisters of St John of God establishing their hospital to care for the people of Geraldton and the surrounding area, the St John of God Hospice are delighted to have coordinated the re-establishment of the share care folder.
We have collaborated with our partners to ensure that the style and content meet the needs of all parties whether the care is being delivered in the home, at a GP or specialist clinic or in the hospice unit”.
Special thanks to the Palliative Care Advisory Committee who purchased the folders, Palliative Care Mid West, General Practitioners and families of patients who offered guidance on the design. We also give special thanks to the artists, Phil Doncon, Ruby McIntosh and Jane Williams Ronan who generously donated the art work for the covers and inside dividers.
Guests at the launch held at the St John of God Specialist Centre included representatives from St John of God Hospital, Geraldton Regional Hospital, Geraldton Palliative Care Service, the artists who generously donated their work and other community members.
