Accreditation

 

The Chief Executive Officer/Director of Nursing is the primary point of contact for any queries by medical practitioners (see contact details below). In her absence you may contact the Executive Assistant - Hospital Management Committee - on 5564 0605.

In order to admit patients and/or operate at the hospital, all medical practitioners must be accredited to practice.

Application packs can be obtained through the Hospital's Chief Executive Officer (see contact details below). Completed application forms should be returned to the hospital and must be accompanied by copies of:

• Primary degree
• Details of specialist recognition (where relevant)
• Medical Practitioners Board of Victoria - Current Renewal of Registration
• Indemnity insurance - Certificate of Currency
• The names and contact details of three (3) referees

The application and all references are taken by the Chief Executive Officer/Director of Nursing to the Hospital's Credentialing Committee for approval. Once approved, temporary or provisional accreditation is provided for twelve (12) months and then reviewed for approval to full accreditation for three (3) years.
