Introducing Health Choices
This service
provides a range of nursing and midwifery care for patients in
their own homes. The service is committed to working closely
with patients, medical practitioners, the extended community and
hospital teams across the metropolitan area of Perth.
Eligibility for the relevant program is
dependent on meeting individual health fund criteria.
Hospital in the Home program
The patient is treated as an inpatient of the
hospital, remaining under the care of their admitting medical
practitioner. The medical team will determine the length of
stay if the medical practitioner believes it is appropriate and
safe for care to be provided in the patient’s home.
Post acute care
This program provides short term (usually 2-6
weeks maximum) ongoing nursing support following discharge
from hospital. Treatments include wound management,
intravenous therapy and stomal therapy.
Postnatal care
The program provides midwifery and lactation
consultant support to assist in the transition to the home
environment. The service offers professional advice, guthrie
testing and monitoring of the progress of the mother and baby.
Service costs
St John of God Health Choices costs are
covered by most health fund providers or can be self funded by the
patient. Referrals to the service can be received from
inpatient wards, emergency department, out patient rooms, medical
practitioners or self referral by patients.
