Accreditation
The Director of Medical Services, Dr Shirley Bowen, is the
primary point of contact for any queries by medical practitioners
regarding credentialling. The contact number is 9311 4060
or e-mail shirley.bowen@sjog.org.au
All medical practitioners and/or allied health professionals who
elect to use the hospital's facilities must apply to the hospital
for accreditation. This includes all specialists and surgical
assistants.
To facilitate your accreditation application, please contact
Medical Credentialling on 9311 4198 or email credentialling.murdoch@sjog.org.au
The credentialling officer will forward you an application
package with information about the process. You are welcome to
receive this information electronically to your e-mail address or
via hard copy or fax, whichever is your preference.
Completed application forms should be returned to the hospital
credentialling officer with all the requested documentary evidence
to facilitate credentialling.
Depending on your category of credentialling, the documentary
evidence required may include certified copies of:
- Primary degree
- Specialist degree
- Details of specialist recognition (where relevant)
- Current CV
- Certified ID (e.g. drivers licence or Passport)
- Current Radiation Council Certificate (if relevant)
- Current laser certificate (where relevant)
- Receipt of payment of current professional registration
- Receipt of payment of current indemnity cover
- Written references from three practitioners within your scope
of practice are required; one of these referees must be accredited
at Murdoch.
- For procedural specialists, additional information relating to
any intended new or innovative procedures also needs to be
provided
- Police clearance
- Working with Children’s check
- A current MRSA screen (taken within the 6 weeks period prior to
your application) must be provided if you have worked or been
admitted to a hospital interstate or overseas within the past 12
months.
Incomplete applications will not be considered by the Hospitals
Credentialling Committee.
The hospital's Credentialling Committee reviews all
applications. Following approval, doctors will be notified by
letter of their credentialling status and scope of
practice.
Accreditation is granted for three years and the recredentialing
process is abridged and requires much less information.
Temporary accreditation may be granted by the Medical Director
for a period up to three months for visiting practitioners, pending
their application for full accreditation being considered by the
credentialling Committee and referees being consulted.
If you have any further questions or require assistance in
commencing practice at the hospital, please contact the Director of
Medical Services, Dr Shirley Bowen on 9311 4060.
We look forward to welcoming you to St John of God Hospital
Murdoch.
