Accreditation process
Application forms can be downloaded
here or obtained from the Credentials Co-ordinator (Hospital
Administration) by calling 9722 1621. Completed forms should
be returned to the Co-ordinator at the address below and must be
accompanied by copies of:
- Primary Degree
- Details of Specialist recognition (where
relevant)
- Receipt of payment of current professional registration
- Receipt of payment of current indemnity cover
- Resume including three referees
All practitioners seeking accreditation must nominate one
emergency back-up. Back-up practitioners need to:
- Be of the same craft group
- Be an accredited practitioner at St John of God Health
Care
- Currently reside and practice in Bunbury while they have
inpatients at the hospital
Subject to initial review, the Chief
Executive Officer may grant three-month Provisional
Accreditation status. The nominated professional referees are
then contacted and requested to submit references.
The Credentialing Committee meets monthly to
consider the credentialing and recommendations of
accreditation. If approved, accreditation may be granted for
periods of up to three years.
