Health services in the home
St John of God Hospital Berwick has partnered
with another St John of God division – Health Choices – to offer a
comprehensive home based nursing service, operating seven days a
week. It is open to all age groups and you do not need to be a
patient at St John of God Hospital to access our services.
Health Choices means a qualified nurse will
visit and treat you in your own homes or another
non-hospital setting.
Our services include:
- Hospital care in the home
- Post acute care
- Community nursing
- Personal care
The type of care you can access through Health
Choices include:
- Healthcare assessments
- Personal care and hygiene
- Medications
- Changing wound dressings
- Post surgery checks
- Co-ordinating other support services
- Health education to both patient and carers
When required, Health Choices will work
alongside existing services in the region including local
hospitals, community hospices, pathology services and wide range of
social outreach programs to ensure a coherent approach to your
care.
Fees
Fees may vary depending on the service you
require. Most health funds will cover all or part of the costs
incurred. Self insured patients should discuss associated fees with
the Health Choices team prior to accessing the service. For
veterans, there is generally no cost incurred. The Health Choices
nursing staff are happy to answer all your questions.
