Social Services offers practical and emotional
support and services to patients, relatives and the hospital
community.
Services offered
- Information regarding community resources
which may be used to improve health, safety or the well-being of
patients, their families and hospital caregivers
- Assistance with discharge planning and
referral to external agencies
- Assist patients and their families with the
Aged Care Assessment process
- Information to patients, relatives and
caregivers regarding financial entitlements, travel and
accommodation options and reimbursements, community services,
support groups etc
All services are confidential and offered

Structure of Social Services
- Includes Continuum Services Manager, Social
Worker, Discharge Planning Team
- Veterans Liaison Officer for DVA
patients
- All transfers to other accommodation, for
example rehabilitation units, Hostels, Nursing
Homes
- All issues related to Aged Care Assessment
and Placement
- Discharge Planning
- Allied Health and Occupational Therapy
Referrals
Residential rehabilitation and outpatient rehabilitation
Discharge planning
- Continuous/ongoing contact with other
agencies regarding referrals
- Access services outside St John of God
Hospital Ballarat
- Work in conjunction with attending doctors,
patients' families and hospital caregivers
- Allied Health referrals to facilitate
appropriate discharge
- Convalescence accommodation
Case management and conferencing
Work with medical professionals, agencies and
allied health to:
- Obtain best outcomes for patients and
families
- Facilitate case conferences and family
meetings when required
- Work with hospital caregivers to facilitate
appropriate outcomes
