St John of God Health Choices offers Community Nursing to individuals directly and through funded programs.

St John of God Health Care has been a provider of Community Nursing services on behalf of the Department of Veterans Affairs since 2001 and currently delivers over 30,000 community nursing services per annum. We also offer the Coordinated Veterans' Affairs program for qualified Veterans.

The CVC Program is a planned and coordinated health care model for eligible Gold Card holders with one or more chronic conditions aDepartment of Veteran's Affairsnd at risk of hospitalisation, in particular those with one or more of the following:

  • congestive heart failure
  • coronary artery disease
  • pneumonia
  • chronic obstructive pulmonary disease
  • diabetes.                                        Coordinated Veterans's Care Program

CVC health care is based around a personalised comprehensive Care Plan.
Care is an ongoing partnership between the CVC participant, their General Practitioner (GP) and a Nurse Coordinator.

Health Choices Ballarat site is also a contracted HACC (Home and Community Care)
Community Nursing and Allied Health provider.

Health Choices also provides community nursing services to agencies involved in the Home and Community Care (HACC) and the Commonwealth Home Care Packages Programs.
