Valued Client Card
The St John of God Health Choices Service
Ethos demonstrates that every encounter, every moment, every person
on every day is valued. The Valued Client Card has been
produced to assist awareness and communication of our relationship
with the client to others.
What is the purpose of the card?
The Valued Client Card is presented to Health
Choices patients/clients to assist them in communicating our
contact details when interacting with others. This could be
to alert hospital caregivers that Health Choices is involved in
care if they are admitted to hospital, to pass on our details to
their GP or to clarify that we are involved for family or
carers.
